The Publishers name, address and contact details
Details of copyright holders - ie for design, illustrations and any photographs.
The ISBN number
Details of the printer
Details of the typesetter / designer
Copyright statements

Page five, being a right hand page, can be used for any dedication by the author. Alternatively, this can be an opening title page. The page backing this, page six, should be left blank.
Page Seven can be used for the Table of Contents which can continue onto its backing page, page eight.
If you have a Foreword or Introduction, this comes next, starting on the next available right hand page.
The opening of your first chapter should always start on a right hand page.
Don't forget to add numbering to each page.
